Lateral Officer Application Materials

Application Packet Materials:        

• Washington State Job Application (MS WORD) WA State Human Resources
• WDFW Personal History and Background Questionnaire (MS WORD)
• WDFW Waiver and Authority for Release of Information
• Veteran’s Preference Eligibility Form (if you are a veteran)

In addition to the application materials listed above, you must also submit a personal resume and a copy of each of the following with your application:

1. Applicant birth certificate;
2. Military discharge certificate of report of separation (form DD214), if you were in the military;
3. Social Security card;
4. Valid driver’s license;
5. Any college degree(s) you may have attained;
6. Abstract driving record (may be obtained from Department of Licensing or Department of Motor Vehicles); and
7. Sealed official college transcript(s)
